Telehealth Informed Consent

This page shows the informational text of the telehealth informed consent. During your intake with our platform partner, you will be asked to review and agree to receive care by telehealth on the terms below before a licensed provider reviews you. Reviewing it here does not, by itself, enroll you or create a provider-patient relationship.
WHAT TELEHEALTH IS: Telehealth uses electronic communications to deliver care without an in-person visit, including asynchronous (store-and-forward) review of the information you provide and, where required, live video.
LIMITATIONS: A remote evaluation may not be appropriate for every condition. Your provider may determine that an in-person visit, lab work, or additional information is required, or that telehealth is not appropriate for you.
YOUR RIGHTS: Your participation is voluntary, you may withhold or withdraw consent at any time, and doing so will not affect your right to future care. The same privacy protections that apply to in-person care apply here.
RECORDS ACCESS: You have the right to access your records and to request copies, consistent with applicable law.
By continuing, I provide informed consent to receive care via telehealth under these terms (Florida Ch. 456.47 and applicable state law).
The version you accept during intake is preserved in your record, so the exact consent you agreed to can be reproduced later.
